Dear CF Retiree: Our open enrollment this year will be from October 25, 2021 through November 5,2021 (10/25/2021 – 11/05/2021) and during this time frame if you would like to make changes to your health insurance plan or cancel, you may do so.
There are no changes to the CF health insurance plans this year. The monthly premiums will be changing effective January 1,2022 plan year. The new rates are included in this packet.
For the upcoming health insurance open enrollment period, CF will continue to offer two health insurance plans to retirees: (1) PPO Blue Options Plan 03769, and (2) Blue Medicare. Both of these plans were offered last year and are compliant under the Affordable Healthcare Act (AHA).
PLEASE NOTE: You may find more affordable health care coverage for yourself or your eligible dependents through the Marketplace by going to http://www.HealthCare.gov. Rates are customized to each individual based upon your personal household situation, such as annual household income, age of those covered, and tax filing status. The Marketplace can help you evaluate your coverage options, including your eligibility for coverage through the Marketplace, and its cost. Note: the plans offered through the Marketplace are not affiliated with CF. If you cancel your health coverage through CF, you will not have the option to re-enroll at a later time.

FBMC Benefits Management will ONLY be responsible for collecting your health/life insurance premium on behalf of the college. All other matters that pertain to your health/life insurance will be handled by the college as it has always been done.
Dependent Eligibility Verification for NEW Dependents – PPO 03769 Participants Only (Does not Apply to Blue Medicare)
If you elect to add new dependents to your health insurance plan (PPO 03769), you will be required to provide documentation showing that they meet eligibility requirements. To verify your dependents, you must submit the required documentation as defined in the Dependent Verification Documentation Chart along with the signed Certificate of Dependent Eligibility Attestation (enclosed). Required documentation must be received in Human Resources by the close of business, November 5, 2021, for the January 1, 2022 effective date of coverage. Your enrollment elections, including the dependents added, are subject to review and/or modification based on the dependent documentation you supply. Please note: this requirement is only for newly added dependents taking effect January 1, 2022. If you have already submitted documents on dependents currently enrolled, you do not have to do it again. The Blue Medicare product is fully insured and Florida Blue has their own process for dependent verification.

As a result of COVID-19 and to protect the health and safety of our retirees and employees, CF will not be holding in-person benefits meetings this year.
Alternatively, we have provided other educational resources created specifically for our retiree group on the CF benefits website.
