An Evidence of Insurability Form is required for: For any increase resulting from a plan or option change that employees elect. Evidence Of Insurability is not required for your Spouse or Child(ren). You can submit an Evidence of Insurability 2 ways:

Requirements for Enrolling in Supplemental Life Insurance at or above 200K
All full-time employees are eligible to add supplemental life insurance coverage or increase the amount of existing coverage. All new requests and requests for coverage of $200,000 or more will require the completion of the Evidence of Insurability form . Coverage is subject to review and not guaranteed. Your enrollment will pend until the form is received in Human Resources.

Please note: this requirement is only for newly added dependents taking effect January 1, 2024. If you have already submitted documents on current dependents to Human Resources, you do not need to do it again.

Rates are customized to each individual based on your personal household situation, such as annual household income, age of those covered, and tax filing status. The Marketplace can help you evaluate your coverage options, including your eligibility for coverage through the Marketplace, and its cost. The Affordable Health Care Exchange Notice provides important information pertaining to the College that you may need when completing an application in the Marketplace.
