CF Voluntary Benefits (Cafeteria Plan)

When you enroll for any voluntary deduction to be paid pre-tax it becomes subject to the IRS regulations. Under the IRS regulations, you may not opt out of any pre-taxed plan during the plan year. You must remain enrolled for the full plan year, unless a qualifying event occurs. Check with human resources to see if your event qualifies.

College of Central Florida's Flexible Benefits Plan includes a Flexible Spending Account (FSA) available through Eagle Benefits. Employees pay for unreimbursed medical expenses and/or dependent care on a pre-tax basis. More information can be found under Flexible Spending Account.

LONG TERM DISABILITY & SHORT TERM DISABILITY

The college offers four (4) options, two STD Plans and 2 LTD Plans.

OneAmerica Long and Short Term Disability plans are offered through Cornerstone Benefits. You may enroll for either or both plans. Newly hired employees may enroll during the first 30 days of employment, no health questions are asked and guaranteed issue. Existing employees, not currently enrolled, may not enroll until the next Open Enrollment period.  If enrolling at a later date, you will be required to answer health questions and coverage is not guaranteed. The plan year runs January 1 - December 31 each year.
Both Long Term and Short Term Disability are post-tax benefits.

Information for Forms the employee will need to complete for filing a claim with OneAmerica for Disability (STD/LTD):

     1. STD/LTD Claim Form
          a. The employee will complete the “Employee’s Statement for Disability Claim form”
          b. The employee will complete the “Authorization for Release of Information form”
          c. CF Payroll will complete the “Policyholder’s Statement for Disability Claim form”
          d. The employee will have their doctor complete the “Attending Physician’s Statement for Disability Claim form”

CANCER INSURANCE (Grandfather Plan)

Allstate Cancer Insurance is offered through Cornerstone Benefits on a pre-tax basis. The plan pays 28 total benefits including: Hospital Confinement, Radiation/Chemotherapy, Surgery, and Transportation. The plan also pays up to $100.00 per year for annual wellness checkups. Benefits are paid directly to the employee and coverage is portable at the same rate guaranteed for life. Employee and Family coverage offered. Newly hired employees may enroll during the first 30 days of employment. Existing employees, not currently enrolled, may not enroll until the next Open Enrollment period. The plan year runs January 1 - December 31 each year.

HOSPITAL SUPPLEMENT - MEDI-GAP HEALTH INSURANCE

Allstate Hospital Supplement-Medi Gap Health Insurance Plan is offered through Cornerstone Benefits on a pre-tax basis.  It was designed to fill the gap between what your health insurance pays and what employees must pay out of pocket toward deductibles and copayments.  There is a “low” plan and a “high” plan. Employees may also cover family members even if they are not on the college health plan. Benefits are paid directly to the employee and coverage is portable. Newly hired employees may enroll during the first 30 days of employment. Existing employees, not currently enrolled, may not enroll until the next Open Enrollment period. The plan year runs FROM January 1 – December 31 each year.
